GENERAL STATEMENT OF DUTIES AND SUPERVISION EXERCISED: The Health Services Coordinator (HSC) is responsible for coordinating resident health care and overseeing medication administration under the direction of the JH Administrator and House RN. The HSC acts as a medication aide when needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provision of quality care to the residents to ensure the optimal level of well-being
Trains medication aides under the direction and supervision of an RN
Works collaboratively with the resident's care team, including pharmacy, psychiatrists, therapists, case managers, guardians, and medical providers
Schedule resident medical appointments and rides
Attends offsite and in-house appointments with residents; briefs other staff who may attend appointments if HSC is not available
Communicate provider ordered changes in medications and treatments to House RN and medical staff
Report medication discrepancies, medication administration compliance issues and resident needs to House RN and/or Administrator
Manage and approve new orders according to current policy and RN instruction
Oversee monthly cycle fill
Maintain inventory of medications and medical supplies. Report needs to House RN and/or Administrator
Document in a timely, concise and accurate manner per policy
Complete monthly progress notes
File documents into resident charts, including scanning into the electronic documentation system
Participate in intake and discharge procedures at the Administrator's instruction
Act independently to problem solve day-to-day resident care issues
Be available for after hours on-call consultation
Negotiates conflicts and resolves problems
Manages residents experiencing crisis situations, provides guidance to staff during crisis situations
Utilizes TeamStepps for communication
Promotes a work environment that promotes retention of staff and where staff input is encouraged, listened to, and responded to
QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
High school diploma or equivalent preferred
Two years of experience providing direct patient care and medication administration preferred
Additional education or experience in mental health settings, psychology, social work, or related fields is desirable
Valid OR driver's license
Pass a criminal background check
Current CPR and Food Handler's certification (or obtained as soon as possible on hiring)
OTHER SKILLS AND ABILITIES
Demonstrates ability and desire to work with people with mental illness; basic understanding of the behavior of people who are under stress
Must have effective communication skills, both oral and written and be able to develop cooperative working relationships with staff, clients, their families, health care providers, administrator and the community
Must be able to maintain relationships with and work collaboratively with health care providers, law enforcement agencies, the court, public and private administrators, and the community in general
Must be computer literate and have the ability to utilize word processing and other software programs
This position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality, and HIPAA regulations

Professional and personal skills to cope with stress associated with work involving a high degree of mental, emotional and physical demands.
While performing tasks of nursing, this position will require fine motor skills and sufficient vision to maintain accuracy.
Use office automation including computer and phone systems that requires fine manipulation, grasping, typing and reaching.
Able to sit for several hours while documenting, auditing, working with clients, reviewing reports and other written materials, and talking on the phone.
This position may occasionally be required to stand; stoop, kneel or crouch, climb or balance, and lift up to 20 pounds.
Infection control and universal precautions must be maintained to ensure disease will not be spread or contracted.
Ability to self-regulate volume of voice to ensure confidentiality and to decrease unwanted stimuli in the RTF.
WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Work is performed in both an office environment and a residential environment. The noise level is usually moderate, but occasionally may be exposed to loud noise such as raised voice levels and alarms. This position may be exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office (i.e., moving mechanical parts, airborne particles, and electrical shock).
